ABSTRACT

Purpose@#This study was performed to introduce an in vivo hybrid multimodality technique involving the coregistration of micro-computed tomography (micro-CT) and high-resolution magnetic resonance imaging (HR-MRI) to concomitantly visualize and quantify mineralization and vascularization at follow-up in a rat model. @*Materials and Methods@#Three adult female rats were randomly assigned as test subjects, with 1 rat serving as a control subject. For 20 weeks, the test rats received a weekly intravenous injection of 30 μg/kg zoledronic acid, and the control rat was administered a similar dose of normal saline. Bilateral extraction of the lower first and second molarswas performed after 10 weeks. All rats were scanned once every 4 weeks with both micro-CT and HR-MRI. Micro-CT and HR-MRI images were registered and fused in the same 3-dimensional region to quantify blood flow velocity and trabecular bone thickness at T0 (baseline), T4 (4 weeks), T8 (8 weeks), T12 (12 weeks), T16 (16 weeks), and T20 (20 weeks). Histological assessment was the gold standard with which the findings were compared. @*Results@#The histomorphometric images at T20 aligned with the HR-MRI findings, with both test and control rats demonstrating reduced trabecular bone vasculature and blood vessel density. The micro-CT findings were also consistent with the histomorphometric changes, which revealed that the test rats had thicker trabecular bone and smaller marrow spaces than the control rat. @*Conclusion@#The combination of micro-CT and HR-MRI may be considered a powerful non-invasive novel technique for the longitudinal quantification of localized mineralization and vascularization.

ABSTRACT

Objective:To evaluate the role of calcium-calmodulin-dependent protein kinase Ⅱ-cyclic adenylate response element binding protein (Ca 2 + -CaMK Ⅱ-CREB) signaling pathway in U50488H-induced reduction of cardiopulmonary bypass (CPB)-caused perioperative neurocognitive disorders in rats. Methods:Forty clean-grade male adult Sprague-Dawley rats, weighing 350-400 g, were divided into 4 groups ( n=10 each) using a random number table method: control group (S group), CPB group (C group), CPB plus by κ-opioid receptor agonist U50488H group (U group), and CPB plus specific CaMKⅡ antagonist KN93 plus U50488H group (K group). Only the arteriovenous catheter was placed in S group, and the blood-free pre-filled cardiac CPB model was established in the other groups.U50488H 1.5 mg/kg was intravenously injected at 30 min before CPB in group U. In group K, 10 μmol/L KN93 5 μl was injected into left lateral cerebral ventricle at 60 min before CPB, and U50488H 1.5 mg/kg was intravenously injected at 30 min before CPB.Morris water maze test was used to assess cognitive function on 3rd day after operation.The rats were then sacrificed, and hippocampal tissues were obtained for determination of the expression of phosphorylated CaMKⅡ (p-CaMKⅡ), phosphorylated CREB (p-CREB) and brain-derived neurotrophic factor (BDNF) (by Western blot) and expression of CaMKⅡ, CREB and BDNF mRNA (by real-time polymerase chain reaction). Results:Compared with S group, the escape latency was significantly prolonged, the number of crossing original platforms was decreased, and the expression of p-CaMKⅡ, p-CREB, CaMKⅡmRNA, CREB mRNA and BDNF protein and mRNA was down-regulated in C, U group and K groups ( P<0.05). Compared with group C, the escape latency was significantly shortened, the number of crossing original platforms was increased, and the expression of p-CaMKⅡ, p-CREB, CaMKⅡmRNA, CREB mRNA and BDNF protein and mRNA was up-regulated in group U ( P<0.05), and no significant change was found in the parameters mentioned above in group K ( P>0.05). Compared with group U, the escape latency was significantly prolonged, the number of crossing original platforms was decreased, and the expression of p-CaMKⅡ, p-CREB, CaMKⅡmRNA, CREB mRNA and BDNF protein and mRNA was down-regulated in group K ( P<0.05). Conclusion:The mechanism by which U50488H reduces CPB-caused perioperative neurocognitive disorders is related to activating the Ca 2 + -CaMK II-CREB signaling pathway in rats.

ABSTRACT

Objective To evaluate the effect of α7 nicotinic acetylcholine receptor (α7nAChR) agonist on inflammasome of NOD-like receptor pyrin domain containing 3 ( NLRP3) during brain injury in-duced by cardiopulmonary bypass ( CPB) in rats. Methods Twenty-four clean-grade adult male Sprague-Dawley rats, weighing 350-450 g, were randomly divided into sham operation group (group S), group CPB, and CPB plusα7nAChR agonist PHA568487 group (group CP) after 5-day Morris water maze train-ing, with 8 rats in each group. Group S was mechanically ventilated for 60 min without receiving CPB. Group CPB received CPB for 60 min. PHA 5684870. 8 mg/kg was intraperitoneally injected at 30 min be-fore CPB in group CP. Water maze test was performed on 3rd day after operation to record the escape laten-cy and times of crossing the original platform. The rats were sacrificed at 2 h after the behavioral test, and their hippocampi were harvested for determination of cell apoptosis ( by TUNEL) and contents of interleukin-1beta ( IL-1β) and IL-18 ( by enzyme-linked immunosorbent assay) , caspase-1 activity ( by using spectro-photometry) , expression of NLRP3, apoptosis-associated speck-like protein containing a CAR ( ASC) and pro-caspase-1 ( by Western blot or real-time polymerase chain reaction) , and expression of NLRP3, ASC and caspase-1 mRNA (using real-time polymerase chain reaction). Apoptotic index (AI) was calculated. Results Compared with group S, the escape latency was significantly prolonged, the times of crossing the original platform were decreased, the AI, contents of IL-1β and IL-18 and caspase-1 activity were in-creased, and the expression of NLRP3 and ASC protein and mRNA, pro-caspase-1 and caspase-1 mRNA was up-regulated in CPB and CP groups (P<0. 05). Compared with group CPB, the escape latency was significantly shortened, the times of crossing the original platform were increased, the AI, contents of IL-1β and IL-18 and caspase-1 activity were decreased, and the expression of NLRP3 and ASC protein and mRNA, pro-caspase-1 and caspase-1 mRNA was down-regulated in group CP (P<0. 05). Conclusion The mechanism by whichα7nAChR agonist alleviates CPB-induced brain injury may be related to inhibiting NLRP3 inflammasome and reducing inflammatory responses in brain tissues of rats.

ABSTRACT

<p><b>OBJECTIVE</b>To establish a public health risk assessment indexes for flood disaster.</p><p><b>METHODS</b>Delphi method and expert consultation were used to establish the public health risk assessment index system and scoring criteria, and the analytic hierarchy process (AHP) method was used to obtain the weight of the indexes. Then, the established index system was applied to evaluate the public health risk of the flood disaster in Anhui province, 2016.</p><p><b>RESULTS</b>A public health risk assessment system consisting of 5 categories and 17 indexes was constructed. The assessment result of the flood disaster in Anhui province of 2016 showed that the public health risk score was 0.26, suggesting that "the health emergency measures were effective, and the public health risks were effectively controlled".</p><p><b>CONCLUSIONS</b>The established indexes can effectively assess the health risk of flood disaster and also can provide the reference for other disaster assessment.</p>

ABSTRACT

Objective To evaluate the relationship between pre-anesthesia/post-anesthesia range of motion (ROM) and knee flexion function after total knee arthroplasty (TKA).Methods 108 patients with osteoarthritis of knee joint replacement performed by the same surgeon of primary TKA screening from 2016 January to 2017 January in our hospital were enrolled in the study.Correlation analysis were conducted between pre-anesthesia/post-anesthesia ROM and knee flexion function of 1,6,12 months after total knee arthroplasty.Results The Correlation coefficient of the Pre-anesthesia ROM and postoperative ROM are 0.392,0.665,0.708 at 1,6 and 12 months,while the Correlation coefficient of the Pre-anesthesia ROM and Post-anesthesia ROM of postoperative are 0.495,0.759,0.793 (P < 0.001),indicating that the correlation difference between pre-anesthesia/post-anesthesia ROM and knee flexion function of 1,6,12 months after total knee arthroplasty is statistically significant (P < 0.05).Conclusions Post-anesthesia range of motion is a better determinant of postoperative range of motion in patients.This should be taken into consideration by surgeons during preoperative planning and in managing patients'expectations.

ABSTRACT

Objective To investigate the recurrence and risk factors of gastric high-grade intraepithelial neoplasia(HGIN)and early gastric cancer(EGC)after endoscopic submucosal dissection (ESD). Methods The clinical and follow-up data on 444 patients(451 lesion)with HGIN and EGC undergoing ESD in Digestive Endoscopy Center of Chinese PLA General Hospital from November 2006 to January 2016 were summarized, and the risk factors of recurrence were analyzed. Results A total of 410 patients were followed-up, and the recurrence rate was 3.2%(13 patients, 13 lesions), with mean recurrence time of 17.6±9.6 months(6-38 months). Univariate and multivariate analysis revealed that the size of the lesion>4.0 cm was the only risk factor of recurrence(P=0.012,OR=10.855,95%CI:1.673~70.442). Conclusion The rate of recurrence is increasing with the EGC extending, therefore, postoperative monitoring should be strengthened to patients with larger lesion.

ABSTRACT

Objective To explore the clinical characteristics of the neuromyelitis optica spectrum disorders (NMOSD) with the area postrema syndrome as the initial symptom.Methods A total of 14 cases were enrolled in the study with the diagnose of NMOSD and the area postrema syndrome as the initial symptom.All the clinical data and imaging profiles by the contrasted magnetic resonance imaging (MRI) of the head and spinal cord were collected and analyzed.Results The median age of onset was (38.1 ± 17.0)years old and the gender ratio of female to male was 10:4.The serum aquaporin-4 (AQP4)-IgG was positive in 11 subjects and several autoimmune antibodies was positive in 7 subjects.The lesions revealed by MRI of the head mainly located in the area postrema and ependymal periphery which often presented as the linear medullary lesion,while linear lesions over three pieces of vertebra were shown by MRI of the spinal cord which mainly in the grey matter and with aH shape around the spinal central canal.Misdiagnose happened in 11 subjects with seven of gastroesophageal reflux disease,two of neurogenic vomiting,one of spinal cord tuberculosis and one of stroke.Conclusions NMOSD should be considered in patients with unexplained intractable nausea,vomiting and/or hiccups lasted for 48 hours or above,especially in those with positive nervous signs.Contrasted MRI and serum AQP4-IgG need to be performed in the suspected patients.Early detection is crucial for patients with NMOSD.

ABSTRACT

Objective To learn the epidemiological characteristics of syphilis in Shihezi in recent three years and to provide bases for prevention .Epidemiological analysis with syphilis was conducted in our hospital from 2012 to 2014 ,which is about the detection rate ,age ,gender ,and the distribution in the department .Methods Serum were deteceted by using three methods and the data were analyzed .Results 1 281 syphilis cases in 74 798 patients were detected in our hospital during this period .The total positve rate of three years was 1 .71% .The positve rate was 1 .35% ,1 .83% and 2 .01% respectively .The results of three years was significant differences(χ2 =39 .877 ,P0 .05) .The ca‐ses were mainly distributed in Han(581 ,45 .36% ) ,Uygur(43 ,3 .36% ) ,Kazak(23 ,1 .80% ) .353 cases were negative by RPR among 1 281 patients with syphilis(27 .56% ) ,and the negative rises year by year .Conclusion The incidence of syphilis increased slightly in Shihezi since 2012 .Though the detection rate between 2013 and 2014 was no significant difference .The focus was on 20- year′s old women of childbearing age .It is necessary to take strict measures to control the spread of syphilis and to do syphilis examina‐tion for all inpatients in order to prevent the infection of syphilis .

ABSTRACT

Objective To evaluate the effect of sevoflurane on the expression of aquaporin 8 (AQP8) in the intestinal mucosa in a pig model of hemorrhagic shock.Methods Twenty-four Bama miniature pigs of both sexes, weighing 22-25 kg, were randomly divided into 3 equal groups using a random number table: sham operation group (group S), hemorrhagic shock group (group HS) and sevoflurane group (group PS).The femoral artery and jugular vein were cannulated for blood pressure monitoring, blood-letting, and blood sampling in anesthetized pigs.Hemorrhagic shock was induced by withdrawing blood from the right femoral artery.Hemorrhagic shock was induced after cannulation in group HS.In group PS, 2% sevoflurane was inhaled for 30 min after the model of hemorrhagic shock was successfully established.Before anesthesia, and at 0.5, 1, 1.5, 2, 3 and 4 h after hemorrhagic shock, blood samples were collected from the jugular vein for determination of serum D-lactic acid and intestinal fatty acid-binding protein (I-FABP) concentrations.The animals were sacrificed at 4 h after hemorrhagic shock, and the intestinal specimens were obtained for microscopic examination and for determination of AQP8 expression in the intestinal mucosa (by enzyme-linked immunosorbent assay).The intestinal water content was calculated.Results Compared with group S, the serum D-lactic acid and I-FABP concentrations, AQP8 expression, and intestinal water content were significantly increased in HS and PS groups (P<0.05).Compared with group HS, the serum D-lactic acid and I-FABP concentrations, AQP8 expression, and intestinal water content were significantly decreased in group PS (P<0.05).The pathological changes of intestinal tissues were significantly reduced in group PS as compared with group HS.Conclusion Sevoflurane can decrease the intestinal mucosal edema through inhibiting AQP8 expression, thus reducing hemorrhagic shockinduced damage to the intestinal mucosa in pigs.

ABSTRACT

Objective To explore impact of BMI on the clinical efficacy of treatment of type 2 diabetes mellitus after the laparoscopic gastric bypass. Methods Twenty patients were randomly selected from type 2 diabetes mellitus patients with BMI of 25 ~28 kg/m2 after laparoscopic gastric bypass surgery in our hospital from 2010 to 2013,20 patients from type 2 diabetes mellitus patients with BMI of 28~35 kg/m2 and 20 patients from type 2 diabetes mellitus patients with BMI≥35 kg/m2 . Accordingly,the total of 60 patients were divided into low BMI group,middle BMI group and high BMI group. The difference of the rate of blood glucose control and other indicators between three groups were compared and analyzed. Results The rate of blood glucose control in low BMI group was 75%,middle BMI group was 85%, and high BMI group was 90%. There was no statistical difference of the rate of blood glucose control between three groups at 12 months after laparoscopic gastric bypass surgery. The blood glucose and weight obviously improved in all patients,and fasting insulin and postprandial ser-um insulin decreased at 12 months after laparoscopic gastric bypass surgery. Conclusion All the patient of type 2 diabetes mellitus with dif-ferent BMI in three group can acquire satisfying clinical therapeutic effect after the laparoscopic gastric bypass.

ABSTRACT

Objective To evaluate the value of bispectral index(BIS)monitoring as an indicator for extubation sedation level after children's oper?ation by total intravenous anesthesia. Methods One hundred and eighty children(2?13 years old)were randomly divided into six groups with 30 cases in each. Group A kept BIS 56?60,and Group B 61?65,Group C 66?70,Group D 71?75,Group E 76?80,Group F 81?85 till extubation. All children were given fentanyl,propofol and cisatracurium besilate when induced,and maintained with propofol and remifentanil. All cases kept BIS 40?55 during the operation. Then the changes of electrocardiogram,mean arterial pressure,heart rate,SpO2,postoperative complications and recovery time were observed. Results Groups A and B were extubated at deep anesthesia,cycle stability,but with high incidence of adverse reactions and awaked with a long time. Groups E and F were extubated when awaked,but with greater stimulation and easier agitation. Groups C and D were lighter hemodynamic responses,less respiratory effects and less postoperative complications of anesthesia. Conclusion The BIS level of 66?75 is a good se?dation level for extubation,especially in the level of 71?75.

Objective To investigate the status on health‐related knowledge and skills among the patients in the Chongqing area and to provide basis for developing public health‐related knowledge and skills .Methods Seven hundred and sixty‐eight patients from 8 general hospitals were sampled ,using the stratified random sampling method .A cross‐sectional investigation on health literacy was conducted with questionnaire“ health awareness survey of patients”,and SAS8 .0 was used to statistically analyze the data .Results The awareness rate of health literacy in Chongqing was 4 .82% .As for the 3 health literacy related aspects including basic health concept and knowledge ,health lif‐estyle and behaviors ,and the health related skills ,the rates were 16 .28% ,2 .21% and 38 .41% respectively .Besides ,the rate of the 5 as‐pects including health conception ,the prevention of infection ,the prevention of chronic diseases ,the safety and the first aid ,the basic medical treatment of analysis were 10 .68% ,20 .31% ,8 .59% ,23 .57% ,10 .68% respectively .There noticed significant differences in different re‐gions ,education ,profession ,the condition of relatives engaged in medical ,medical insurance category(P<0 .05) .Profession and the condition of relatives engaged in medical were the main influencing factors on the level of health‐related knowledge and skills among the patients(P<0 .05) .Conclusion The prevalence of basic health‐related knowledge and skills is low in Chongqing city and we should make comprehensive interventions to improve health liter of patients .

ABSTRACT

AIM:To investigate whether mitochondrial membrane potential (ΔΨm ) and the mitochondrial ap-optotic pathway are involved in the protective mechanism of Panax quinquefolium saponin ( PQS) against cardiomyocyte ap-optosis after ischemia/reperfusion ( I/R) injury in rat myocardium .METHODS: Ninety healthy male SD rats were ran-domly divided into sham group , I/R group, PQS (200 mg· kg -1 · d-1 ) +I/R group, cyclosporine A ( CsA) group, CsA (10 mg· kg-1 ) +I/R group and PQS +CsA +I/R group.The model of myocardial I/R injury in vivo was established by ligating the left anterior descending artery ( LAD) for 30 min followed by 120 min of reperfusion in the rats .The serum ac-tivity of lactate dehydrogenase ( LDH ) was measured by automatic chemistry analyzer .The myocardial infarct size was measured by Evans blue and 2,3,5-triphenyltetrazolium chloride (TTC) staining.Cardiomyocyte apoptosis was detected by in situ TDT-mediated dUTP nick end labeling (TUNEL).The protein levels of Bcl-2, Bax, cleaved caspase-3 and cytosol-ic cytochrome C were determined by Western blotting .ΔΨm was measured by laser scanning confocal microscopy and fluo-rescence microplate reader .RESULTS:Compared with I/R group, the serum content of LDH ,the infarction size in PQS+I/R group, CsA+I/R group and PQS +CsA+I/R group and the myocardial apoptotic index were decreased .Compared with I/R group, the fluorescence intensity of mitochondria after JC-1 staining was enhanced in PQS +I/R group, CsA+I/R group and PQS+CsA+I/R group, and the relative fluorescence units (RFU) ofΔΨm were improved in those 3 groups. In PQS+I/R group, CsA+I/R group and PQS+CsA+I/R group, the protein expression of Bcl-2 was increased, and that of Bax was decreased compared with I/R group.Moreover, in those 3 groups, the protein levels of cleaved-caspase-3 and cytosolic cytochrome C were decreased compared to I /R group, respectively.CONCLUSION:PQS attenuates myocardial injury and cardiomyocyte apoptosis during I /R, and the protective mechanisms of PQS were associated with the modulation ofΔΨm and the inhibition of mitochondrial apoptosis pathway .

Objective To investigate the expression and clinical significance of mineral dust-induced gene(MDIG) in malignant pleural effusions (MPE) and tuberculosis pleural effusions (TBPE).Methods Fifty-four patients with MPE (MPE group) and 50 patients with TBPE (TBPE group) were collected.The MDIG protein was detected by enzyme-linked immunosorbent assay and the relative quantification of MDIG mRNA was measured by real-time quantitation polymerase chain reaction.The cutoff value,sensitivity and specificity of the MDIG protein and MDIG mRNA to diagnose the MPE were evaluated by receiver operating characteristic (ROC) curve analysis.By Spearman correlation analysis,the correlation of MDIG protein and MDIG mRNA was evaluated.Results The MDIG protein in MPE group was significantly higher than that in TBPE group [(304.38 ± 228.47) ng/L vs.(44.43 ± 40.57) ng/L],and there was statistical difference (P < 0.01).The MDIG mRNA in MPE group was significantly higher than that in TBPE group (6.27 ± 3.54 vs.1.82 ± 0.64),and there was statistical difference (P < 0.01).With a cutoff point of 114.23 ng/L,MDIG protein had a sensitivity of 77.8% and a specificity of 94.0% for differential diagnosis.With a cutoff point of 2.75,MDIG mRNA had a sensitivity of 88.9% and a specificity of 92.0% for differential diagnosis.There was a positive correction between MDIG protein and MDIG mRNA (r =0.915,P < 0.01).Conclusions The MDIG protein and MDIG mRNA are highly expressed in MPE with a good sensitivity and specificity.MDIG protein and MDIG mRNA maybe a good clinical indicator in the diagnosis of malignant pleural effusions.

ABSTRACT

Objective To investigate what kind of obese patients appropriate to adopt the laparoscopic adjustable gastric banding volume reduction surgery.Methods A retrospective study was performed to review the clinical data of 40 patients who required reoperation to remove the gastric banding after LAGB from November 2003 to March 2013 at the Department of Minimally Invasive Surgery,Changhai Hospital.Selected 40 patients who required LAGB from January 2006 to October 2008 as control group.We conducted a case-control study to analyze.Chi-square test and multivariate and non-conditional Logistie regression analysis were used to identify the risk factors of removing of gastric banding.Results Age and gender were not statistically significant different (P > 0.05).Multiple factors of Logistic regression showed that BMI≥35 kg/m2,postoperative clinic visits per year < 3 and on the basis of gastrointestinal disease were risk factors for the removal of gastric banding (Wald =3.908,7.375,5.209,P < 0.05).Conclusion The risk factors for the removal of gastric banding include BMI,postoperative clinic visits and the basis of gastrointestinal disease.In the treatment of obesity with LAGB should take full account of the above factors.

ABSTRACT

The combination of nanotechnology and molecular imprinting technique, including their application research in biomedical domain, provides a new solution to the problem of the substitutes for antibodies, enzymes, and other native biological structures as well as cell bracket materials. Nanocavity biomaterials with recognition specificity imprinted by using proteins as templates have numerous applications in biotechnology, medicine and so on. This review presents the aspects of molecular imprinting nanostructure involved in the biomacromolecules imprinting, and it explores the precent developments and achievements of nanomaterials for molecular imprinting technology.

ABSTRACT

Objective To screen the difference of gene expression in dorsal root ganglia (DRG)of inflammatory visceral hypersensitivity rats and to explore the role of voltage gated calcium channel (VGCC) in inflammatory visceral hypersensitivity. Methods Total 180 male Sprague-Dawley rats were in this study,the weight varied from 200 gram to 300 gram. 2,4,6-trinitrobenzenesulfonic acid (TNBS) model group was maken by 2. 0% TNBS slowly injection,the dosage was 100mg per kilogram. The normal control group was only injected with same volume of 0. 9% sodium chloride solution. After the model had been maken for four days,gene expression profiles of L6-S2 DRGs were tested by rat cDNA microarray chips. And the result was verified by RT-PCR and Western blot. The changes of intracellular Ca2+ and the voltage gated calcium currents were recorded by patch-clamp.The special Ca2+ channel blockers were given by intrathecal injection,and then the changes of visceral sensitivity were observed. The visceral sensitivity was measured by abdominal withdrawal reflex (AWR). Results There were significant changes of 172 genes expression in L6-S2 DRGs of TNBS model rats,which included Ca2+ channel,membrane receptor and intracellular second messenger. Of those,L-type Ca2+ channel (Cav1. 2) and R-type Ca2+ channel (Cav2. 3) were significantly up-regulated. The results of gene microarray chips were further confirmed by RT-PCR and Western blot.The intracellular Ca2+ testing indicated that there was no statistical significant of resting intracellular Ca2+ in colonic special sensory neuron between TNBS group and normal control group (P>0. 05);while the evoked transients [Ca2+] significantly increased compared with normal control group (P<0. 05). The whole cell patch clamp recording showed that the L-type and R-type calcium current were significantly increased in colonic primary sensory neurons of TNBS group compared with normal control group (P<0. 05). The inflammatory visceral hypersensitivity was significantly reduced by intrathecal injection of nimodipine and SNX-482 (P<0. 05). Conclusion The up-regulation of Cav1. 2and Cav2. 3 play an important role in inflammatory visceral hyperalgesia,which may be the possible potential therapeutic targets for visceral inflammatory hyperalgesia.

ABSTRACT

Objective To investigate the effect of propofol on the brain injury in children undergoing open heart surgery under cardiopulmonary bypass (CPB).Methods Thirty children aged 4-10 yr undergoing repair of ventricular septal defect or atrial septal defect under CPB were randomly divided into 2 groups ( n = 15 each):control group (group C) and propofol group (group P). Anesthesia was induced with iv injection of sufentanil 1 μg/kg, propofol 2.5 mg/kg (midazolam 0.2 mg/kg in group C) and pipecuronium 0.1 mg/kg in group P. Anesthesia was maintained with iv infusion of 1% propofol 6 mg·kg-1 ·h-1 (0.05% midazolam 0.2 mg·kg-1 ·h-1 in group C) until the end of operation. Isoflurane ( 1%-2% ) were inhaled during the operation in all patients. Sufentanil 1 μg/kg and pipecuronium 0. 1 mg/kg were injected intravenously after the start of CPB. A catheter was inserted into right internal jugular vein and advanced cephalad until jugular bulb for blood sampling before CPB, at 30 min of CPB, at the end of CPB, at 30 min after termination of CPB, at the end of operation, and at 24 h after termination of CPB to detect the plasma concentrations of 8-iso-PGF2 and S-100β by ELISA. Arterial and jugular bulb venous blood samples were collected for determination of SjvO2 before CPB, when naso-pharyngeal temperature was reduced to 30 ℃, when naso-pharyngeal temperature returned to 36 ℃ and at the end of CPB. Cerebral arterial venous oxygen content differences ( Da-jvO2 ) and cerebral oxygen extraction rate of oxygen ( CEO2 ) were calculated. Results Plasma concentrations of 8-iso-PGF2α and S-100β were significantly lower, SjvO2 was significantly higher, Da-jvO2 and CEO2 were significantly lower in group P than in group C (P < 0.05=. ConclusionPropofol can reduce the brain injury in children undergoing open heart surgery during CPB and the mechanism is related to decrease in cerebral oxygen metabolic rate and lipid peroxidation.

ABSTRACT

Normal function of growth hormone-insulin-like growth factor Ⅰaxis is essential for linear growth after birth. A case of continuous growth with undetectable growth hormone level even under insulinhypoglycemia stimulation was reported. The growth hormone deficiency was due to pituitary stalk interruption combined with deficiency of multiple pituitary hormones. Taken together with reviewed literature, this so-called nongrowth hormone-dependent linear growth was preconditioned by other hormones, especially gonadotropin deficiency,and the unclosed epiphysis.

ABSTRACT

Objective To investigate the diagnosis of Crohn's disease(CD)in Ruijin hospital of School of Medicine of Shanghai Jiaotong University according to the criteria made by World Health Organization(WHO)and Chinese Medical Association.Methods The consecutive patients with CD diagnosed in Ruijin hospital between 1998 and 2007 were retrospectively analyzed.The clinical manifestation.endoscopic,radiologic and pathologic findings of all patients were re-evaluated accordingto the diagnostic criteria of CD proposed by WHO and Chinese Medical Association in 2007.The diagnostic level of CD was assessed.Results Only 5 and 10 patients were confirmed to be well consistent with the criteria of WHO and Chinese Medical Association,respectively.Two hundred and thirty-six patients were confirmed as suspected diagnosis to the criteria of Chinese Medical Association and 20 patients to WHO.The level of endoscopic diagnosis was raised,wherease the level ofpathologic diagnosis was decreased in recent years compared with that before 2004.Conclusion The pathologic diagnosis rate of CD is still at a low level.The current diagnostic level of CD is lagging behind the demand of the guidelines.
